Note: Physical application and notification form(s) shall be submitted by hand or by post to the Intermediaries Team, Supervision Division of the Mandatory Provident Fund Schemes Authority at Level 12, Tower 1, The Millennity, 98 How Ming Street, Kwun Tong, Hong Kong. Form RP - SI(I) shall be emailed to intermediaries@mpfa.org.hk.
